Minimum qualifications: Doctor of Medicine Degree or Doctor of Osteopathic Medicine Degree from an accredited medical school; successful completion of a family medicine or internal medicine residency program; board certification in family medicine or internal medicine, a current, unrestricted license to practice medicine in the state of Mississippi; experience in caring for patients with IDD in a residential setting with severe and profound levels of disability; current registration certificate issued by the Drug Enforcement Administration (DEA). Contract will be for a period of one year and contractor will be compensated at a rate of $1,400.00 per month. Contractor will work for up to 10 hours per week. WHEREAS, on the December 16, 2016, Eric D Sullivan and Marinda Sullivan, executed a Deed of Trust to Scott R.
